Is Gynecomastia Surgery Painful?

Gynecomastia surgery should not cause sharp pain during the procedure because the chest is thoroughly numbed with anaesthesia. At Transform Plastic Surgery Clinic, most gynecomastia operations are performed under local anaesthesia. The patient remains awake and may notice pressure, movement or pulling, but the treated area should remain numb.

After surgery, patients commonly experience soreness, tightness and tenderness rather than severe pain. These sensations are temporary and can usually be managed with prescribed medication and appropriate aftercare.

Which Anaesthesia Do We Use?

Most gynecomastia procedures at Transform Plastic Surgery Clinic are performed under local anaesthesia. This approach allows the patient to remain awake while the chest is carefully numbed.

General anaesthesia is also available when the patient prefers to remain asleep, provided it is medically suitable following the required anaesthetic assessment.

The final choice considers:

  • The patient’s preference and anxiety level

  • The extent of fat and gland removal

  • Relevant medical conditions

  • The surgeon’s and anaesthesia team’s safety assessment

What Happens on the Day of Surgery?

The operation usually takes approximately 90 minutes, although the duration varies according to the amount of fat, glandular tissue and skin requiring correction.

Patients typically spend around four hours at the facility, including preparation, surgery, observation and discharge. Gynecomastia surgery is normally performed as a day-care procedure, so most patients can return home the same day after being assessed by the clinical team.

When surgery is performed under local anaesthesia without sedation, prolonged fasting is generally unnecessary, and the patient can eat according to the clinic’s instructions before and after surgery. Patients undergoing general anaesthesia—or sedation when applicable—must follow the fasting instructions provided by the anaesthesia team.

How Long Does the Local Anaesthesia Last?

Our local-anaesthesia protocol can continue providing pain relief for approximately 18–20 hours after administration, although the exact duration varies between patients.

This means the chest may remain numb or relatively comfortable for much of the first postoperative day. Soreness may become more noticeable as the anaesthetic gradually wears off. Patients should begin prescribed pain medication according to their instructions rather than waiting for discomfort to become severe.

What Does Recovery Feel Like?

Once the local anaesthetic wears off, the chest may feel:

  • Sore or tender

  • Tight or stiff

  • Swollen

  • Sensitive around the incisions

  • Similar to muscle soreness after a heavy chest workout

Severe or progressively increasing pain is not considered routine and should be reported to the surgeon.

When Can I Resume Normal Activities?

Most patients can walk, eat and perform basic personal activities on the day of surgery. Light routine activities can generally be resumed the following day if the patient feels comfortable.

However, returning to normal daily activity does not mean that the chest has fully healed.

Patients should:

  • Avoid strenuous exercise and chest workouts

  • Avoid lifting heavy objects for approximately four weeks

  • Wear the compression garment as advised

  • Avoid driving while impaired by sedation, strong pain medication or restricted arm movement

  • Resume strenuous work or gym training only after clearance from the surgeon

Patients with physically demanding occupations may require more time away from work than those performing desk-based duties.

Final Answer

At Transform Plastic Surgery Clinic, most gynecomastia procedures are performed under local anaesthesia. The operation usually takes about 90 minutes, and the patient typically spends around four hours at the facility before returning home the same day.

The local anaesthetic may remain effective for approximately 18–20 hours. After it wears off, some soreness, tightness and tenderness are expected, but severe pain is uncommon and should be assessed. Light daily activities can usually resume the next day, while heavy lifting and strenuous exercise must be avoided for approximately four weeks.

General anaesthesia remains an option for patients who prefer to be asleep, subject to appropriate medical and anaesthetic assessment.

Timings and recovery differ between patients. Individual instructions provided by the treating surgeon take priority over general online guidance.
